Homebased Outreach Support Services are delivered in the service-users preferred environment. Our primary intervention is the Collaborative Recovery Model. This is a one to one service, a working alliance is established between the service-user and the Recovery Support worker with the express focus on the individuals recovery goals. This service is available in the Shires of East Gippsland, Wellington, South Gippsland and Bass Coast, with service outlets located in Bairnsdale, Sale and Leongatha.
Psychosocial Rehabilitation Day Programs begin by engagement in Collaborative Therapy. Opportunities for social inclusion and participation in a range of community based activities are available with the level of support required by the individual. Day Programs are available in the Shires of East Gippsland and Wellington from service outlets located in Bairnsdale and Sale.
Prevention and Recovery Care (PARC) services are delivered to the Gippsland Region in partnership with Latrobe Regional Hospital – Mental Health Services from a residential facility in Bairnsdale. SNAP’s primary intervention in PARC is an adapted version of Collaborative Therapy which is currently being researched by Frameworks for Health (see research). PARC services are voluntary and are aimed at preventing admissions to acute mental health facilities and to assist in a staged return to community life after an acute admission. This is a 10 bed short-stay unit (7-28 days) that operates 24 hours per day 7 days per week.
