Senior Recovery Support Worker

General Information:

 SNAP Gippsland is committed to excellence, equity and quality in mental health.  It achieves this by forming partnerships that generate hope, meaning, self-identity, and responsibility.

 SNAP was established in 1992 and is a leader in Psychiatric Disability Rehabilitation Support services in Gippsland. It has a history of delivering programs that offer a point of difference. SNAP has adopted two research validated models of service as the agency’s method of intervention. The model known as the Collaborative Recovery Model (CRM) has been developed by the University of Wollongong with whom SNAP has established a partnership agreement. The model known as Collaborative Therapy (CT) has been developed by the Mental Health Research Institute and is used primarily in SNAP’s Day Programs.

SNAP assists people with mental health problems to manage their own symptoms and motivates them to set goals, achieve dreams and lead fulfilling and rewarding lives.

SNAP delivers Homebased Outreach Support Services, Psychosocial Rehabilitation Day Programs Respite services and Prevention and Recovery Care (PARC) services

Service outlets are located in Bairnsdale, Sale, and Leongatha.

SNAP is governed by a community based Board of Management, and receives the majority of its funding from the Department of Human Services.

Terms of Employment: This position is for a 3 year term, a three-month probationary period and satisfactory performance appraisals linked to agreed outcomes that will be conducted at three months and thereafter every 12 months.

 Accountability: The position is accountable to and under the direction of SNAP Gippsland’s General Manager.

Key Responsibilities:
  1. To be responsible for the delivery of regular, consistent, service that focuses on recovery and encompasses the   essential elements of hope, meaning, self-identity and responsibility.
  2. To be well informed and up to date in regard to SNAP’s protocols, policies and procedures.
  3. To engage, include, encourage and develop a working alliance with participants.
  4. To work together with participants on their recovery journey by:
    • assisting participants to identify their recovery vision.
    • carrying out participant needs assessments, collaborative goal setting and measurement of outcomes
    • acting as a link into the broader community by encouraging participants to access the wider social, educational, employment and  recreational structures
    • advocating on behalf of participants as required.
  5. Liaise with other relevant service providers, Latrobe Regional Hospital – Mental Health Services, Community Health services, employment services, alcohol and other drug services, respite services, carer support services etc….where appropriate to develop referral pathways, ensure continuity of care and the provision of appropriate and complimentary services.  This may include participation in case conferences, development of collaborative program plans etc.
  6. Team work:
    • Communicating effectively
    • Working collaboratively and supporting co workers and other staff members
    • Participating in meetings
    • Exploring different approaches and contributing to new initiatives
    • Working through difficult crisis situations with others in accordance with the  program’s approach, individual Care Plan specifications and relevant policies
    • To be available if necessary out of regular hours for on call/ crisis management
  7. To maintain the Quarterly Data Collection requirements, undertake administration tasks including recording client information etc…
  8. Participate in regular supervision
  9. Actively pursue professional development training relevant to the program and philosophy of SNA
  10. Participate and assist in the update and maintenance of policies and procedures and strategic planning processes.
  11. To carry out other duties as negotiated.
Key Selection Criteria:

To be successful in this position, applicants must be able to demonstrate the following:

  2. A strong personal philosophy that reflects the elements of Recovery.
  3. A clear understanding of mental health issues and of the disabilities associated with mental illness.
  4. Ethical work practices and good personal boundaries.
  5. A sound knowledge of available community resources.
  6. Well developed organisational skills, flexibility and the capacity to adapt to a changing work environment.Commitment and proven ability to work collaboratively within teams, organizations and with other agencies.
  7. Resilience, creativity and a positive “can do” approach. Capacity to work independently where necessary.
  8. Excellent verbal and written communication skills. Computer literacy in Microsoft Windows XP based applications (Word, Email, Internet etc…)
  9.  A working knowledge of the relevant legislation and service standards, such as the Mental Health Act 1986, National Mental Health Service Standards, Psychiatric Disability Support Service Standards, the Information Privacy Act, Health Records Act and Psychiatric Disability Rehabilitation and Support Services Guidelines for Service Delivery.
  10. A current Victorian driver’s licence.
 Qualifications:

 Tertiary qualifications in Social Work, Psychology, Occupational Therapy, Community Development, Nursing or related field of practice

Knowledge and experience in human services delivery preferably in psychiatric disability. Experience in working with clients with complex presentations including trauma, substance abuse, other disabilities, homelessness, personality disorders and systemic disadvantage.

 Additional Requirements:

A declaration of pre-existing health issues and any pre-existing work cover matters.

A Police check is mandatory and will be required prior to an appointment being made.
