General Information:

SNAP was established in 1992 and is a leader in Psychiatric Disability Rehabilitation Support services in Gippsland.

It has a history of delivering programs that offer a point of difference. SNAP has adopted two research validated models of service as the agency’s method of intervention. The model known as the Collaborative Recovery Model (CRM) has been developed by the University of Wollongong with whom SNAP has established a partnership agreement. The model known as Collaborative Therapy (CT) has been developed by the Mental Health Research Institute and is used primarily in SNAP’s Day Programs.

SNAP assists people with mental health problems to manage their own symptoms and motivates them to set goals, achieve dreams and lead fulfilling and rewarding lives.
SNAP delivers Homebased Outreach Support Services, Psychosocial Rehabilitation Day Programs Respite services and Prevention and Recovery Care (PARC) services
Service outlets are located in Bairnsdale, Sale, and Leongatha.

SNAP is governed by a community based Board of Management, and receives the majority of its funding from the Department of Human Services.

The establishment and development of the Prevention and Recovery Care (PARC) Service is a collaboration between SNAP Gippsland Inc and Latrobe Regional Hospital Mental Health Services.

The PARC service is underpinned by the principles of PREVENTION, RECOVERY & CARE as per the guidelines set down by the Department of Human Services, Mental Health Branch.

The PARC service facility is located at Moroney Street, Bairnsdale. This is a 10 bed residential rehabilitation and recovery facility, for adults with mental health problems requiring short-term stays of up to 28 days.

Service Aim:

In line with the Department of Human Services, Mental Health Branch guidelines, the aim of the PARC service is to expand the range of service options to:

  • people with a severe mental illness in the inpatient setting and in the community whose treatment and recovery is better suited to intensive, short-term treatment and support in a residential setting.
  • prevent admissions and avoidable readmissions to inpatient units and supplement crisis intervention services.
  • provide quality clinical and psychosocial rehabilitation services that focus on prevention and assist in recovery, delivered in a safe and hopeful environment
  • establish care pathways that can be sustained beyond discharge from the PARC service.
  • minimise the trauma and disruption for consumers and their carers that may arise from a first episode or relapse of mental illness.
Target Population:

The service will be available to people living in Gippsland aged 18 -64 who are:

  • eligible for adult area mental health services
  • at risk of an acute inpatient hospitalization, who require a more intensive level of support than can be provided in their own home in order to prevent risk of further deterioration or relapse
  • no longer require acute inpatient level of care and treatment and would benefit from short-term intensive treatment and support in a residential setting post discharge from the acute facility
Terms of Employment:

The position is of a casual nature. As PARCS is a 24 hour 7 day per week service rotational shifts apply. These include morning, afternoon and evening shifts and public holidays. Casual staff will be called upon when there are staff absences.

Accountability:

The position is accountable to and under the direction of the SNAP Gippsland’s General Manager and PARC service Co-ordinator.

Key Responsibilities:

To be responsible for the delivery of regular, consistent, service that focuses on recovery and encompasses the essential elements of hope, meaning, self-identity and responsibility.

To be well informed and up to date in regard to the protocols, policies and procedures relevant to PARCS.

To engage, include, encourage and develop a working alliance with participants.
To work together with participants on their recovery journey by:

  • assisting participants to identify their recovery vision.
  • carrying out participant needs assessments, collaborative goal setting and measurement of outcomes
  • acting as a link into the broader community by encouraging participants to access the wider social, educational, employment and recreational structures
  • advocating on behalf of participants as required.

Work in a spirit of partnership with Latrobe Regional Hospital – Mental Health Services staff. Liaise with other relevant service providers, Community Health services, employment services, alcohol and drug services, respite services, carer support services etc….where appropriate to develop referral pathways, ensure continuity of care and the provision of appropriate and complimentary services. This may include participation in case conferences, development of collaborative program plans etc.

To maintain the Quarterly Data Collection requirements.

Participate and assist in the update and maintenance of policies and procedures and the strategic planning processes relevant to PARCS.

Undertake administration tasks including recording client information etc…

To carry out other duties as negotiated.

Key Selection Criteria:

To be successful in this position, applicants must be able to demonstrate the following:
A genuine respect and commitment to improving the quality of life of people with mental health problems. This encompasses an understanding of the Social Model of Health.

A strong personal philosophy that reflects the elements of Recovery.

A clear understanding of mental health issues and of the disabilities associated with mental illness.

Ethical work practices and good personal boundaries.

A sound knowledge of available community resources.

Well developed organisational skills.

Excellent verbal and written communication skills. Computer literacy in Microsoft Windows XP based applications (Word, Email, Internet etc…)

Commitment and proven ability to work collaboratively within teams, organizations and with other agencies.

A working knowledge of the relevant legislation and service standards, such as the Mental Health Act 1986, National Mental Health Service Standards, Psychiatric Disability Support Service Standards, the Information Privacy Act, Health Records Act and Psychiatric Disability Rehabilitation and Support Services Guidelines for Service Delivery.

A current Victorian driver’s licence.

Qualifications:

HIGHLY DESIRABLE:

An appropriate tertiary qualification in a health related area.
Experience in working with and supporting people with a psychiatric disability resulting from a serious mental illness.

ADDITIONAL REQUIREMENTS:

A declaration of pre-existing health issues and any pre-existing work cover matters.
A Police check is mandatory and will be required prior to an appointment being made.
