SNAP is a Gippsland based Psychiatric Disability Rehabilitation Support Service (PDRSS) that was established in 1992. With service outlets in Bairnsdale, Sale, and Leongatha SNAP delivers a range of home-based outreach support (HBOS), planned respite, and day program services that are tailored to meet local service needs. In addition SNAP manages the bed-based Gippsland Prevention and Recovery Care (PARC) service located in Bairnsdale in partnership with Latrobe Regional Hospital-Mental Health Services.
SNAP is highly regarded for its service delivery framework that is a true recovery based model and features a structured approach that is tailored to respond to the individual’s stage of recovery. The framework combines several components that are well recognised as “Good Practice” within the sector. SNAP has established strong partnerships with universities and research organisations to adopt and further ‘practice-develop’ these model components using an evidence-based approach.
SNAP’s strong links with research organisations and its well developed service model are core strengths that enable SNAP to take a lead role in policy and service development in the Gippsland region and beyond.
